Each designated block represents a separate subcontractor performing work under the Building <br /> Permit. <br /> b. Complete all required contractor information in the designated blocks including RME information and <br /> signature. <br /> c. Fill in Electrical and Plumbing contractors'phases of work to be performed in the"Phase(s)section." <br /> (Reference list attached). <br /> d. If additional contractors are part of the project use additional Page 2 forms as needed. <br /> B) Electrical or Plumbing only applications: <br /> (The Project Declaration is required at the time of application.) <br /> 1)Electrical and Plumbing contractor to complete Page 1 of the Project Declaration Form. <br /> 2) TMK <br /> 3) Project Location <br /> 4) Declare Electrical or Plumbing Work. <br /> 5) Provide Electrical or Plumbing valuation of work(Your contract amount). <br /> 6) Complete Contractor information(Responsible Managing Employee (RME), signature). <br /> 7) Electrical or Plumbing contractor to fill in Part 2 of the Project Declaration Form. <br /> a. Each designated block represents each electrical and plumbing subcontractors performing work <br /> under the Permit. <br /> b. Complete all required contractor information in the designated blocks including RME information and <br /> signature. <br /> c. Fill in Electrical and Plumbing contractors'phases of work to be performed in the"Phase(s)section." <br /> (Reference list attached). <br /> d.
