NOTICE OF NONPROFIT 6 MONTH PROGRESS REPORT All recipients of the County of Hawaii Nonprofit Grants -In -Aid for FY 2021-2022 must submit a six-month progress report within thirty (30) calendar days after December 31 of the 2021-2022 fiscal year. DEADLINE FOR SUBMISSION: Submitted online via Google Forms no later than January X 2022, at 4:30 PM. A link to the form can be found on the Department of Finance webpage, under the Budget tab as "Nonprofit Grant Forms". Partial reports will not be accepted, and there will be no further notice. REPORTING REQUIRED: To fulfill reporting requirements, you must provide as follows: • Program and contact information. • Grant award total and percentage claimed to date. • Updated Income Budget Form (a link to the form is provided on the webpage). • Updated Expenses Budget Form (a link to the form is provided on the webpage). • If applicable, please indicate the total amount of funds requested for the next grant payment. • List quantitative progress towards meeting outcome measures outlined in the program's Logic Model. • In 1,500 characters, explain program progress relative to its timetable and describe next steps. In addition, indicate any issues that may have impacted the development and implementation of the program. • A photo accompanied with a short narrative to illustrate progress towards meeting outcome measures and the public benefits derived from use of the County Grants -In -Aid funds. • SUBMIT via Google Forms to Progress Report FY 21-22 - Google Forms.
