SUMMARY <br /> Applicant/Owner: Hamakua Health Center, Inc., a Hawaii non-profit corporation, doing <br /> business as Hamakua-Kohala Health <br /> Property Location: The Property is located along and makai of the Akoni-Pule Highway. <br /> Located within the vicinity of the Property are Kohala Hospital (east), <br /> Maluhia Road(west) and Kamehameha Park, and St. Augustine Episcopal <br /> Church abuts the Property's eastern boundary. The Property's address is <br /> 54-3793 Akoni Pule Highway, Kapaau. <br /> Tax Map Key: (3) 5-4-005:011 (por.) ("Property") <br /> Request: To establish the Hamakua-Kohala Health Center and accessory structures <br /> and uses (collectively,the "Project")within an approximately+3.822-acre <br /> portion of the 5.682-acre parcel consisting of the following components: <br /> • Transitional Clinic, a single-story, 4,153 square feet building and <br /> accessory structures and uses within the State Land Use ("SLU") <br /> Agricultural District and County Agricultural (A-20a) zoning district, <br /> pursuant to a Special Permit, as permitted under HCC § 25-5-72(d)(7) <br /> and HRS § 205-6; <br /> • Permanent Clinic, a two-story, 23,973 square feet building and <br /> accessory structures and uses within the SLU Urban District and <br /> County Single-Family Residential (RS-15) zoning district,pursuant to <br /> a Use Permit, as permitted under HCC § 25-5-3(b)(8). In addition, <br /> existing Building C is proposed to be utilized initially as the <br /> caretaker's residence, and may also be utilized for storage, an office or <br /> other related uses accessory to the Transitional Clinic and Permanent <br /> Clinic; and <br /> • Workforce Housing, a two-story, 5,696 square feet, 8-bedrooms/ <br /> 8-baths building and accessory structures and uses for varying mixes <br /> of Project contract workers and employees,traveling multi- <br /> disciplinary researchers and trainees working at the Project,within the <br /> SLU Agricultural District and County Agricultural (A-20a) zoning <br /> district,pursuant to a Special Permit under HRS § 205-6. <br /> i <br />
