■ Evaluation of REACH Ashland Youth Center(County of Alameda) <br /> Serving as project manager for this multi year evaluation since 2076. Designed and conducted annual evaluation plans <br /> to assess youth participant demographics,participation levels,quality of programming,and outcomes for collaborative- <br /> ly-run youth center serving an unincorporated area of Alameda County with high levels of poverty, incarceration rates, <br /> unemployment, and violence.Annual evaluation activities include developing, coordinating administration of, and an- <br /> alyzing annual youth survey(including piloting survey questions with youth);cleaning and analyzing demographic and <br /> participation data from the Kid Trox database and Excel-based tracking tool used by the Coordination of Services TEAM <br /> (COST);and presenting evaluation highlights in visually engaging deliverables. Inquiry questions addressed through <br /> the evaluation in specific years include understanding the care coordination system, exploring longer-term impacts of <br /> REACH program participation on youth,assessing how the REACH partnership is functioning related to various collab- <br /> orative competencies, documenting how parents and caregivers of REACH members perceive how programming has <br /> impacted youth. <br /> ■ Evaluation for Cal istoga Community Schools Initiative(UpValley Family Centers) <br /> Serving as project manager for this multi year evaluation. Developed evaluation plan for a cradle-to-career collective <br /> impact initiative working with low-income Latino immigrant families. Clean and analyze deidentified student data(in- <br /> cluding matching participation data with academic records and mental health assessment tools)and produce an annual <br /> evaluation reports. <br /> ■ Process Evaluation of the Community Health Improvement Plan's Implementation/Partners for Health Santa Clara <br /> County Collaborative(Santa Clara County Public Health Department) <br /> Co-facilitated the process evaluation for Partners for Health SCC,a cross-sector collaborative supported by the Public <br /> Health Department to implement the County Community Health Improvement Plan(CHIP). Using a collective impact <br /> approach, the evaluation included providing technical assistance and training for collaborating organizations,conducting <br /> a Collaborative Partnership survey and key leader interviews to explore partners'motivation for continued engagement, as <br /> well as Partners for Health SCC's early achievements. <br /> ■ Clipper START Evaluation(San Francisco Metropolitan Transportation Commission) <br /> Coordinating survey and focus group data collection for means-based fare pilot program involving an increasing num- <br /> ber of transit operators serving the Bay Area's nine counties. <br /> ■ Santa Clara County Transportation Needs Assessment <br /> Designed and managed outreach plan to identify specific opportunities and challenges related to addressing transporta- <br /> tion needs for six hard-to-reach populations served by various County agencies:individuals with serious mental illnesses <br /> and/or medical conditions who are accessing services through a variety of County Departments, adults on probation, <br /> victims of crime and non-risk criminal witnesses, and youth served by the child welfare/foster care system.Analyzed and <br /> presented community engagement findings and helped develop and refine targeted recommendations for the County <br /> Executive's Office and Board of Supervisors. <br /> ■ Santa Clara County Children's Health Assessment <br /> Managed all aspects of conducting and analyzing 79 focus groups and 30 key leader interviews that achieved sampling <br /> plan goals. Wrote report of qualitative findings, including sections on Structural Racism, Economic Inequities, and Edu- <br /> cation. Facilitated small group discussions at community meetings and summarized action forum conversations. <br /> ■ San Mateo County Community Collaboration for Children's Success Neighborhood Planning Processes(San Mateo <br /> County Health System) <br /> Managed all aspects of the planning process for the CCCS initiative, including development of four neighborhood plans <br /> with extensive community engagement, facilitation of a cross-sector and County-wide steering committee, and coordi- <br /> nating closely with subconsultants and neighborhood champions. This engagement included the development,admin- <br /> istration,and analysis of surveys and focus groups with youth 73-24 years old,parents and other caregivers for children <br /> and youth 0-24 years old in four languages(including the collection of consent and assent forms and processing of <br /> incentives).
