r <br /> Affidavit of Trustee of the <br /> ZIEBARTH FAMILY TRUST <br /> 1. The following Trust(the "Trust) is the subject of this Affidavit: <br /> ROBERTA J. ZIEBARTH and SUSAN STRAUSLIN, Trustees, or their <br /> successors in trust, under the ZIEBARTH FAMILY TRUST, uta dated <br /> September 29, 2015 <br /> EIN: 81-6622366 <br /> 2. On September 29, 2015, WILLIAM F. ZIEBARTH and ROBERTA J. <br /> ZIEBARTH executed the ZIEBARTH LIVING TRUST. The ZIEBARTH <br /> LIVING TRUST, dated September 29, 2015, provides that upon the death of the <br /> first spouse to die, the trust assets will be divided into two separate sub-trusts <br /> called the ZIEBARTH FAMILY TRUST and the ZIEBARTH MARITAL <br /> TRUST. <br /> 4. WILLIAM F. ZIEBARTH, Trustor and Trustee, died December 10, 2015. <br /> 5. The ZIEBARTH LIVING TRUST has now been split into the ZIEBARTH <br /> FAMILY TRUST and the ZIEBARTH MARITAL TRUST. Both of these trusts <br /> are currently in full force and effect, and are governed by the provisions of the <br /> ZIEBARTH LIVING TRUST. <br /> 6. Attached to this Affidavit and incorporated herein by reference are selected <br /> provisions of the Trust evidencing the following: <br /> a. Creation of the ZIEBARTH Living Trust and Initial Trustees <br /> b. Successor Trustees <br /> C. Signature Pages <br /> d. Provisions pertaining to Division of assets into the <br /> ZIEBARTH FAMILY TRUST and the ZIEBARTH MARITAL <br /> Trust <br /> e. Death Certificate of WILLIAM F. ZIEBARTH. <br /> 7. The Trust provisions which are not attached to this Affidavit are of a <br /> personal and confidential nature and set forth the distribution of Trust <br /> property.
