REGISTRATION PROCESS CONT. <br /> The applicant will also need to submit the following information to register a TAR: <br /> • A registration form that includes the name and contact information for the property owner or <br /> operator if different from the owner; <br /> • A site plan showing the location of all structures on site and the requisite parking; <br /> • A floor plan showing the location of the rooms for rent; <br /> • A notarized affidavit in the format determined by the Director that the TAR meets health, safety and <br /> code requirements; <br /> • A maximum renter limit calculation; <br /> • Verification that State of Hawaii general excise tax are in effect; and <br /> • Verification of the County of Hawaii real property tax clearance certificate. <br /> • If applicable, the owner will be required to submit a copy of their valid real estate broker's license or <br /> real estate salesperson's license. <br />
