ADA Coordinator <br /> Procedure for Requesting Modifications <br /> Page 3 of 4 <br /> AFTER A REQUEST IS MADE <br /> The ADA Coordinator shall determine if the individual who made the <br /> request or the individual on whose behalf the request was made is an <br /> individual with a disability and is eligible to use the facility, program, or <br /> service in question. Individuals who make requests but are not qualified to <br /> receive the modification shall be so informed.2 <br /> If the individual who made the request is qualified, the ADA Coordinator <br /> shall assess the request to determine if it is reasonable and notify the <br /> appropriate department head or designated departmental staff of the <br /> request. If the request is reasonable, the ADA Coordinator shall work with <br /> the department to establish a timeline for implementation of the request. <br /> The ADA Coordinator shall notify the individual who made the request of <br /> its status either verbally or in writing, and when known, the expected date <br /> of implementation. <br /> REQUESTS THAT ARE NOT REASONABLE <br /> Requests for modifications shall be considered on a case-by-case basis. <br /> The County is not required to implement modifications which are not <br /> reasonable. Modifications are not reasonable if they: <br /> ■ Fundamentally alter the nature of the program, service, or <br /> procedure offered by the County; <br /> ■ Are extremely costly and/or disruptive, or would create an <br /> administrative burden to implement; or <br /> ■ Pose a significant risk of substantial harm or a direct threat to the <br /> health or safety of the individual or others that cannot be <br /> reduced or eliminated by a reasonable modification. <br /> When a requested modification will not be implemented because it is <br /> unreasonable, the ADA Coordinator or designated employee handling <br /> the request shall contact the individual to explore if there are alternatives <br /> that would provide the requestor an equal opportunity to participate or <br /> access the County facility, program, or service. If the ADA Coordinator <br /> and the requestor cannot identify an acceptable alternate modification, <br /> the ADA Coordinator shall consult with others who have demonstrated an <br /> ability to solve problems and exercise sound judgment with respect to <br /> 3 <br />
