f _ <br /> ATTACHMENT B-PAST PROJECT EXPERIENCE <br /> Below is a short list of projects completed within the last year throughout the State of Hawaii. All of these <br /> projects were completed without error to both client and regulatory agency's satisfaction. Detailed descriptions of <br /> recent projects are as follows: <br /> • Hazardous Materials Survey and Abatement Air Monitoring— Hawaii Community College Campuswide <br /> Repairs <br /> Conducted hazardous materials surveys for asbestos-containing materials, lead painted surfaces, arsenic- <br /> containing building materials, mercury-containing building materials and PCB-containing materials prior to <br /> the various renovation projects occurring at various classroom buildings and office buildings throughout the <br /> Hawaii Community College campus. Developed hazardous materials abatement plans for the renovation <br /> project. Abatement air monitoring was conducted during hazardous materials disturbance per the Hawaii <br /> Department of Health requirements. <br /> • Mold Surveys and Remediation, Hazardous Materials Surveys, Soil Surveys and Remediation, Abatement <br /> Design and Abatement Air Monitoring for Various Renovation and Demolition Projects—State of Hawaii <br /> Department of Education <br /> Conducted mold surveys and remediation, soil surveys and remediation, hazardous materials surveys and <br /> abatement activities for various renovation projects at the Hawaii DOE public schools throughout Hawaii <br /> island and the State. Directed and planned informative meetings regarding hazardous materials abatement <br /> and contaminated soil remediation, regulatory requirements and safety protocol with concerned staff and <br /> public members. The hazardous materials surveys and abatement included lead paint, asbestos, arsenic, <br /> mercury, and PCBs. Soil surveys included metals, pesticides, petroleum and various other suspect <br /> contaminants. Provided hazardous materials abatement activities including subcontracting abatement <br /> companies, providing project supervision, air monitoring, and consultation services. Developed abatement <br /> design specifications for proper handling, removal and disposal of all identified hazardous materials and <br /> identified contaminated soils remediation. <br /> • Hazardous Materials Surveys, Assessment and Planning for County Facilities — County of Hawaii, <br /> Department of Public Works <br /> Conducted hazardous materials surveys, damage assessments and abatement activities at the County of <br /> Hawaii Department of Public Works facilities island wide. The hazardous materials surveys included lead <br /> paint, asbestos, arsenic, mercury, and PCBs. Provided abatement activities including subcontracting <br /> abatement companies,providing project supervision,air monitoring, and consultation services. <br /> • Phase I Environmental Site Assessments for Various Land Acquisitions—Hawaii Department of Land and <br /> Natural Resources,Division of Forestry and Wildlife <br /> Successfully completed numerous Phase I Environmental Site Assessments for various land acquisitions for <br /> The Hawaii DLNR Division of Forestry and Wildlife on the island of Hawaii, Oahu, Maui, Molokai and <br /> Kauai in accordance with the USA EPA AAI Final Rule (40 CFR Part 312) and ASTM Standard Practice for <br /> ESA (ASTM E1527-13). <br /> • Hazardous Materials Surveys,Mold Survey,Abatement Design and Abatement Air Monitoring for Various <br /> Renovation and Demolition Projects—Hilo Benioff Medical Center <br /> Conducted mold surveys and remediation, hazardous materials surveys and abatement activities for various <br /> renovation projects for the Hilo Benioff Medical Center. The hazardous materials surveys and abatement <br /> included lead paint, asbestos, arsenic,mercury, and PCBs. Provided hazardous materials abatement activities <br /> including subcontracting abatement companies, providing project supervision, air monitoring, and <br /> consultation services.
