Takase <br />Director <br />SUBJECT: Application No. 25-09, Itsuko's LLC dba Itsuko's Kitchen/Banzai 47, for an <br />original Restaurant General liquor license, Category B2, located at 1550 Kamehameha Avenue, <br />Hilo, Hawaii, 96720; TMK: 2-2-032:010. <br />NATURE OF APPLICATION: This is an application for an original Restaurant General <br />liquor license, Category B2. <br />CATEGORY B2: Category B2 allows the licensee to provide recorded music, live <br />entertainment, and dancing within its licensed premises. <br />APPLICANT: The applicant, Itsuko's LLC, is a domestic limited liability company, registered <br />with the Department of Commerce and Consumer Affairs, State of Hawaii, on January 12, 2023. <br />It is manager -managed with its managers as follows: <br />Mandy Castro — 51 % share holder <br />Matthew Castro- 49% share holder <br />The name of the registered agent in the State of Hawaii is as follows: <br />Mandy Castro, P.O. Box 318, Papaikou, Hawaii 96781 <br />PERSONAL HISTORY: For information relative to the personal history of the applicant, <br />please refer to Exhibit 1 and Exhibit 2 attached. <br />LOCATION OF PREMISES: The proposed licensed premises is located at <br />1550 Kamehameha Avenue, Hilo, Hawaii, the former location of JKL & Associates, LLC <br />dba E Komo Mai Diner Bar & Grill (CAB-811). For further information please refer to Exhibit <br />3 attached. <br />DETAILS OF PREMISES: The proposed licensed premises is located in a single -story, <br />wooden structure consisting of approximately 6,930 square feet. Within the structure there is a <br />certified kitchen and storage room, main dining room, meeting room, bar, and another larger <br />tatami room which Applicant plans to use for overflow and/or private parties. There is also an <br />enclosed uncovered lanai / designated smoking area attached to the bar side of the premises. <br />Restrooms for both men and women are located within the proposed licensed premises.
