About 25.8 percent of the 2,514 unsheltered homeless persons identified in the 2409 point -in- <br />tirne (PIT) count were determined to be chronically homeless. HUD deftn.es the chronically . <br />homeless to be_- An unaccompanied individual with a disabling condition, who has either been <br />continuously homeless for at least one (I) year OR has had at least four (4) episodes of <br />homelessness in the past three (3) years. A disabling condition is defined by HUD as a <br />diagnosable substance use disorder, serious mental illness, developmental disability, or chronic <br />physical illness or disability, including the co- occurrence of two or more of these conditions. In <br />addition, a person roust be sleeping in a place not meant for human habitation (e.g., living on the <br />streets) or in an emergency shelter during the entire period(s) of homelessness.9 <br />Table IV-9. Approximately 400 households received HOP WA assistance in the 2007 <br />Program year, leaving an estimated gap of more than 600 households in need of housing <br />assistance. <br />A survey of 161 people with HIV in Hawaii indicated a rate of 74 percent in need of <br />housing assistance (63 percent long term and I 1 percent short term). Applying that <br />proportion of need to the 2,700 confirmed number of people with HIV, results in a need <br />for nearly 2,000 housing units. Approximately 400 households received HOPWA <br />assistance in the 2007 program year, leaving an estimated gap of 1, 600 households in <br />need of housing assistance. (This is likely a high estimate since the survey was <br />administered through service providers which likely missed people who are not in need of <br />services.) <br />The HOP W-4 planning group identified the housing needs of this population which included <br />permanent housing (rental subsidy), assisted living, transitional housing, emergency housing, <br />long --term care facilities, care homes and hospice care.
