West Nile Virus <br /> Dead Bird Collection Form <br /> Person Completing Form: <br /> Name: Phone: <br /> Address: <br /> Date collected: <br /> Address or other specific location where bird was found (town. city. street, road, etc.) <br /> How To Collect Dead Birds <br /> The Center for Disease Control advises that there is no danger of contracting West Nile Virus from handling dead <br /> birds. However,the following procedure is suggested to keep hands clean when collection a dead bird <br /> 1. If possible, use rubber gloves when picking up dead birds. If gloves are not available, insert <br /> hand into a plastic bag and use the bag as a glove (grocery bag is fine). <br /> 2. Place each bird in a plastic bag (sandwich bag,food storage bag, grocery bag) and tie or seal <br /> shut.Then place bag inside a second bag and tie or seal it shut. <br /> 3.
