(b) Format. <br /> (1) Form and Size. Any paper or document filed with the Commission shall be <br /> written in ink,typewritten,mimeographed or printed; shall be plainly legible; <br /> and shall be on strong, durable paper no larger than 8-1/2" x 11" in size, <br /> except that maps, charts,tables, and other similar documents may be larger, <br /> folded to the size of the papers to which they are attached. <br /> (2) Title and Identification. Any document or paper shall show the title of the <br /> proceeding before the Commission and the name and address of the party or <br /> the party's representative. <br /> (3) Signature. The original of each document must be signed in ink by the party <br /> or the party's duly authorized representative. The signature of the person <br /> signing the document constitutes a certification that that person has read the <br /> document;that to the best of that person's knowledge,information, and belief, <br /> every statement contained in the document is true and no such statement is <br /> misleading; and that it is not interposed for delay. <br /> (c) Copies. Unless otherwise specifically provided by these rules, an original and ten <br /> copies of each document shall be filed. <br /> (d) Defective Filing. If any document filed in a proceeding is not in substantial <br /> conformity with the applicable rules of the Commission as to the contents thereof, or <br /> is otherwise insufficient,the Commission's designated representative shall return the <br /> document and require correction of the deficiency. If a document is returned,the <br /> document shall be effective as of the date of receipt of the corrected document. <br /> (e) Amended Document. A document or pleading may be amended at any time prior to <br /> hearing. An amendment offered prior to hearing shall be served on all parties and <br /> filed with the Commission. All parties shall have the opportunity to answer and be <br /> heard on an amendment filed after hearing commences, and the chairperson shall <br /> decide whether such amendment shall be allowed. <br /> (f) Extension of Time. Whenever a party is required to file a document within the <br /> period prescribed or allowed by these rules, by notice given thereunder or by order or <br /> rule,the chairperson may(1) for good cause before the expiration of the prescribed <br /> period,with or without notice to the parties, extend such period; (2)pursuant to a <br /> stipulation between all of the parties, extend such period; or(3)permit the act to be <br /> done after the expiration of a specified period where the failure to act is clearly <br /> shown to be the result of excusable neglect. Any request for a continuance, except <br /> for stipulations, should be made by written motion,unless the request is made during <br /> the course of a hearing. <br /> (g) Retention of Documents by the Commission. Any document filed with or presented <br /> to the Commission shall be retained in the files of the Commission. The chairperson <br /> may permit the withdrawal of an original document upon submission of properly <br /> authenticated copy to replace such document. <br /> 2-5 Continuance or Extension of Time. <br /> 7-8 <br />
