assistance from public or private agencies that address housing, health, social services, <br />employment, education, or youth needs. <br />The County does not have a formal policy for individuals being discharged from publicly funded <br />institutions and systems of care. These individuals would normally fall under the State's jurisdiction and <br />we as a County would follow the State's policy. However, under the County's Tenant Based Rental <br />Assistance (TBRA) program, preference is given to applicants who are homeless, victims of domestic <br />violence, aged -out foster care youths and families living in a transitional housing program. <br />Discussion <br />In respect to persons who are not homeless but have other special needs, the County is partially <br />funding, Phase II, of an elderly rental project, that is being developed by Hawaii Island Community <br />Development Corporation ( HICDC), with HOME funds. The Mohouli Neighborhood Heights Senior <br />Housing Project will be a 120 unit elderly rental project located in Hilo, Hawaii. Phase I of the project <br />consisting of 60 units, began in April, 2012 and was completed in March, 2014. Phase II of the project <br />will consist of 30 units and is scheduled to begin in May, 2016 and completion in May, 2017. The County <br />is providing nutritional and transportation services to the residents. Hawaii Island Adult Care, Inc. (HIAC) <br />is working with HICDC to relocate their program on -site of the project. The HIAC would provide an adult <br />day care program to the residents, as well as to other seniors in the community. The HIAC program <br />would further the health of the seniors by keeping them engaged, physically and mentally involved, and <br />in the presence of trained personnel that can respond to their needs. The HIAC provides an adult day <br />care program where elders attend for the day, they have dignity, spend time with peers in activities of <br />interest and are a part of the community. In the past, the County has provided CDBG funds for the <br />planning, design, site and infrastructure improvements for the HIAC project. In 2015, CDBG funds were <br />provided for foundation and site improvements. 2016 CDBG funds will be used for the vertical <br />construction of the facility and is scheduled to start in September, 2016 and completion in September, <br />2017. Part of the 2016 CDBG funds will be allocated to the Mobile Homeless Outreach Service Program, <br />which is administered by HOPE Services Hawaii Inc., and the CDBG funds will be used to acquire two <br />vehicles to assist in the outreach to homeless individuals. The Kama kana Villages -Senior Affordable <br />Housing - Infrastructure Improvements is another project that will be partially funded with 2016 CDBG <br />funds. The CDBG funds will be used for infrastructure improvements for the development and <br />construction of 85 units for a Senior Housing Project in Kailua -Kona. <br />One year goals for the number of households to be provided housing through the use of HOPWA for: <br />Short -term rent, mortgage, and utility assistance to prevent homelessness of the <br />individual or family <br />Tenant -based rental assistance <br />Units provided in housing facilities (transitional or permanent) that are being <br />developed, leased, or operated <br />Annual Action Plan <br />2016 <br />OMB Control No: 2506 -0117 (exp. 07/31/2015) <br />34 <br />
