What are extremely hazardous substances and hazardous chemicals? <br /> Extremely Hazardous Substances (EHS) are those substances and quantities listed at 40 CFR part <br />355, Subpart D, Appendix A (alphabetical order) and Appendix B (CAS order) <br /> Hazardous chemicals are any substances for which a facility must maintain a MSDS under the <br />OSHA Hazard Communication Standard, which lists the criteria used to identify a hazardous <br />chemical. <br />What chemicals are excluded? <br /> Any food additive, color additive, drug or cosmetic regulated by the Food and Drug <br />Administration; <br /> Any substance present as a solid in any manufactured item to the extent exposure to the <br />substance does not occur under normal conditions of use; <br /> Any substance to the extent it is used for personal, family, or household purposes, or is present <br />in the same form and concentration as a product packaged for distribution and use by the <br />general public; <br /> Any substance to the extent it is used in research laboratory or a hospital or other medical <br />facility under the direct supervision of a technically qualified individual; <br /> Any substance to the extent it is used in routine agricultural operation or is a fertilizer held for <br />sale by a retailer to the ultimate customer. <br />How do I report? <br />Download the information package on filing your annual Hawai`i Chemical Inventory Form (HCI)/Tier2 <br />Form. In addition, the HDOH HEER Office encourages facility operators to use the Tier2Submit software <br />package and provide the HEER Office with an electronic version of your data. The Tier2Submit software <br />was developed by the EPA and the National Oceanographic and Atmospheric Administration (NOAA) and <br />can be downloaded from the EPA website. <br /> Information Package for Hawaii Chemical Inventory Form (HCIF)/Tier II \[PDF\] <br /> (State Agency) Information Package for Hawaii Chemical Inventory Form (HCI)/Tier II \[PDF\] <br /> EPA Tier2Submit Software <br />When are facilities required to report? <br />An owner or operator that is required to report shall submit a HCI/Tier II Form before March 1 of each <br />year for the previous reporting year. For example, HCI/Tier 2 forms for the reporting year January 1, <br />2010 through December 31, 2010 must be submitted by March 1, 2011. <br />Who do I report to? <br />The HCI/Tier 2 Form shall be submitted annually to the HDOH HEER Office, your LEPC, and the Fire <br />Department having jurisdiction over the facility. <br />Is there a filing fee? <br />The fee is $100.00 per facility and is made payable to the State of Hawai`i HEER Office. The filing fees <br />are deposited into an account within the Environmental Response Revolving Fund for further <br />disbursement to the LEPC accounts. <br /> <br />
