With these data trends in mind, we will do our best to maintain the range of services along the <br />aging continuum and ensure that our services are consumer -centered and culturally responsive. <br />To do this, we will rely on collaborative partnerships and on evidence -based models. We will <br />track our progress using nationally -recognized data indicators and we are confident that our <br />seamless system of care will continue to make the Hawaii County a great place to live, work, <br />and play for older adults and people with disabilities. <br />Based on the Administration on Aging's (AoA) initiatives, the State Executive Office on Aging <br />(EOA), Area Agencies on Aging (AAA), and Hawaii County's unique community and geographical <br />makeup, we are pursuing the following goals for this planning period: <br />well <br />Goal 1.Age Well: Maximizing opportunities for older adults to age well, <br />remain active, and enjoy quality lives while engaging in their <br />communities. <br />Goal 2. Forge Partnerships: Forging partnerships and alliances that will <br />give impetus to meeting Hawai'i's greatest challenges of the aging <br />population. <br />Goal 3. Enhance the ADRC: Developing a statewide ADRC system for <br />older adults and their families to access and receive Long Term Support <br />Services (LTSS) within their respective counties. <br />Goal 4. Live at Home with Dignity: Enabling people with disabilities and <br />older adults to live in their community through the availability of and <br />access to high-quality Long Term Services and Supports, including <br />supports for families and caregivers. <br />Goal 5. Keep Kupuna Safe: Optimizing the health, safety, and <br />independence of Hawai'i's older adults. <br />
