3.0INTRODUCTION/PURPOSE <br />The purpose of this supplemental hazardous materials survey was to investigate the Subject Site for the <br />presence of ACM, lead painted surfaces, mercury-containing lamps and PCB-containing light ballasts <br />not previously identified in the Previous Report for the Subject Site. Specifically, LEI completed the <br />following tasks: <br />Reviewed the April 2008 Hazardous Materials Survey report completed by EKNA Services, <br />Inc.; <br />Performed site reconnaissance at the Subject Site; <br />Visually inspected all accessible areas of the Subject Site; <br />Collected three (3) samples of suspect ACM from the Subject Site; <br />Submitted the thirty (3) samples of suspect ACM to Hawaii Analytical Laboratory, LLC for <br />analysis of asbestos via Polarized Light Microscopy (PLM) in accordance with the AHERA <br />protocol and NIOSH Method 600/R-93/116; <br />Collected three (3) paint chip samples from the Subject Site; <br />Submitted the three (3) paint chip samples to Hawaii Analytical Laboratories, LLC for analysis <br />via EPA Method 7000B for total lead content; <br />Inspected the Subject Site for suspect mercury-containing light bulbs and PCB-containing light <br />ballasts; and <br />Prepared this report documenting the field activities and the results of the survey, including <br />analytical results, photographs, recommendations and maps. <br />Supplemental Hazardous Materials Survey October 23, 2017 <br />Keaau Fire Station <br />4 <br /> <br />
