condition or circumstances of a particular <br />individual, whether or not a participant; <br />c. OHCD's evaluation of recorded or unrecorded <br />information about a particular participant; <br />d. Medical, psychological, or psychiatric data, <br />including diagnosis and history of disease or <br />disability of a particular participant; <br />e. Correspondence concerning a particular <br />participant; and <br />f. The name of the worker or unit in which the case <br />is or was active. <br />2. The use or disclosure of any of the information <br />specified in Section (D1) shall be limited to the <br />following persons or purposes and, unless otherwise <br />stated, this excludes inspection of their case record: <br />a. Disclosure for purposes directly connected with <br />the administration of housing assistance programs. <br />(1) Only the employees of the OHCD and HUD in the <br />performance of the employees' official duties <br />shall have access to the entire case record. <br />(2) Disclosure shall be allowed to the extent <br />necessary to provide services and to <br />determine eligibility or amount of assistance <br />for participants identified in Sec. C. <br />(3) Under the provision of Section D (2) (a), <br />disclosure to the following agencies shall be <br />permitted: <br />(i) Social service agencies from which the <br />participant receives services and, <br />(ii) Agencies administering programs under <br />the Social Security Act. <br />(4) When disclosure to individuals or agencies <br />other than those specified in Section <br />D(2)(a)(1) is necessary and permitted under <br />Section D(2)(a)(2) and the participant has a <br />choice as to whether disclosure is to be made <br />(does not include situations such as fraud, <br />Rules and Regulations <br />-6- <br />0010ARslh-TBRA <br />11/01/2007 <br /> <br />
