(a) The name of the individual authorized to <br />receive the information or to review the <br />record, and the individual's OHCD <br />connection, if any; <br />(b) The purpose for which the information is <br />being sought; <br />(c) Participant's social security number or <br />birth-date, and address; and <br />(d) Specific period of time the <br />authorization is valid, not to exceed <br />ninety days. <br />(2) Reproduced copies of information requested <br />shall be provided at costs established in HRS <br />§92-21 at not less than five cents per copy. <br />Copies of rules shall be charged at ten cents <br />per page, or as otherwise established in HRS <br />§ 91.2.5. Actual postage costs shall be <br />charged. <br />Disclosure to owners or their designated <br />agents of Federally-funded projects under <br />this paragraph shall be permitted only for <br />participants of Federally-funded projects and <br />only to facilitate the provision of services <br />or financial assistance to the participants. <br />E. DISCLOSURE OF INFORMATION TO PARTICIPANTS <br />1. Case records shall be reviewed only in designated areas <br />within the OHCD's office. The records shall not be <br />removed from the premises. <br />2. Information from records shall be released to the <br />participant or legal guardian of the participant upon <br />the individual's request provided that a signed and <br />dated written request is received. <br />3. When the information requested contains or consists of <br />coded or abbreviated material, such as computer input <br />and input and output forms, the OHCD shall provide, if <br />requested, translations of the codes or abbreviations. <br />4. Reproduced copies of information requested shall be <br />provided at costs established in HRS §92-21 at not less <br />than five cents per copy.
