ATTACHMENT
CONTRACT MODIFICATIONS:
Effective September 29,2020,the parties mutually agree that the CONTRACTOR shall

continue to provide the required services with the following modifications: R
1. Attachment-S 1, Scope of Services, subparagraph B.2, is modified to read as follows:

2.Provide and maintain participant, fiscal, statistical,and administrative records
pertaining to services as specified by the STATE, including but not limited to
purchasing and maintaining a Licensing Agreement with the Collaborative Planning
Group Systems,Inc.or any other Licensor specified by the STATE. ;

2. Attachment-S2, Compensation and Payment Schedule
a, The total amount of compensation is increased by"$110,000.00" from
"$505,101.07" to "$615,101.07" of federal funds.
b, The Budget, attached to the Contract as Exhibit"O",is hereby deleted and
replaced with a revised Budget,attached hereto as Exhibit"P" and made a part
hereof.In addition,the CONTRACTOR shall submit a STATE approved
detailed Budget no later than fifteen(15)days after the execution of this
modification, and failure to comply may result in the withholding of payments to
the CONTRACTOR.Upon submission of a STATE approved detailed Budget,
the STATE approved detailed Budget shall become part of Exhibit"P" and
made a part of this Contract.
3. Attachment-S3,Time of Performance
In accordance with paragraph 4 of Attachment-S5, Special Conditions,the
performance period of the Contract is extended by an additional twelve(12months)
from September 30,2020,to and including September 29,2021.
4. All other terms and conditions of the Contract shall remain the same.
