Dear Mr. Waltjen:
Enclosed is the fully executed Agreement for the following Project:
Project Title: Victim Assistance Program (VOLA)
Project Period: July 1, 2021 to June 30, 2022
Federal Award: $1,004,892 (Federal Funds)
Administrative Funds: $ 1,000
Agency Match: $ 251,223 (20% Matching Contribution)
Total Project Cost: $1,257,115
The continuation of funding beyond the initial 12-month period is contingent on the project's
satisfactory performance, meeting the contract's requirements, and the availability of funds.
Additionally, satisfactory performance includes fulfilling fiscal and programmatic requirements
that include obligating the VOCA and agency matching funds and providing the victims services
(as specified in the contract) during the contract period. An unsatisfactory performance will
impact future VOCA funding unless improvements are immediately made.
Enclosed are the Project Self-Assessment and Subgrant Award Report(SAR) forms. Please
submit both forms by September 28, 2021. The Project Self-Assessment helps us to understand
your agency's experience managing similar grants. The SAR is required by the Office of Victims
of Crime(OVC) for all subgrantees that receive funding, and helps us collect basic information
on subgrantee recipients and the program activities that will be implemented with VOCA plus
match funds.
