Re: Resolution for multi-year agreement
Enclosed is a resolution authorizing the payment of funds for more than one fiscal year,
for a multi-year agreement with Ventiv Technology, Inc. for software maintenance and
user support. The Department of Human Resources, Workers' Compensation Division
currently utilizes Ventiv Claims software for workers' compensation claims management.
The Department would like to enter into a five (5)year agreement to secure lower
pricing. Annual fee for year one is estimated at $17,094 with gradual yearly increases
ending in year five at approximately $19,239.

A. REQUEST:
Approval to enter into a five (5) year software maintenance and user support agreement with Ventiv
Technology, Inc. (formerly David Corporation).
E. BACKGROUND AND JUSTIFICATION USE ADDITIONAL SHEETS AS NEEDED
The Department of Human Resources Workers' Compensation Division currently utilizes Ventiv Claims
software(formerly called NavRisk)for workers' compensation claims management. To leverage a lower cost
for maintenance and user support, we would like to enter into an agreement for five years. The fee for each
contract year will be billed on an annual basis, totaling $17,093.28 for year one, $17,606.08 for year two,
$18,134.26 for year three, $18,678.29 for year four, and $19,238.64 for year five, The total cost for the five
year contract will be$90,750.55.
