Subject: Papaikou Wastewater Treatment Plant
Operation and Maintenance Inspection - EMD/WB 991015
The County of Hawaii Wastewater Division has received the annual Operation and
Maintenance Inspection Report for the Papaikou Wastewater Treatment Plant and
provides the following corrective action plan to address the deficiencies cited in the
report.
Deficiencies:
1. The Chlorine Contact Tank contained a 6 to 12 inch layer of sludge on the
bottom of the tank.
The sludge layer in the Chlorine Contact Tank was removed. Monitoring of the
sludge layer in the Chlorine Contact Tank is normally performed on a regular
basis but has now been added to the Operator's monthly Preventive
Maintenance Checklist as a weekly check.
2. The Chlorine Alarm was inoperable.
The Wastewater Division has purchased a new Chlorine Alarm system for the
facility. However, the new equipment was received in a damaged condition and
has been returned to the factory for replacement. Operators currently utilize a
portable Chlorine Gas detector at the facility for monitoring of Chlorine Gas.
The new Chlorine Alarm will be installed upon receipt and monthly testing of the
new Chlorine Alarm system will be added to the Operator's monthly Preventive
Maintenance Checklist. Expected completion date is January 31, 2000.

3. The headworks ventilation fan was inoperable.
The Wastewater Division has received a replacement fan and is currently in the
process of installing the replacement fan. Modification of the assembly is
required since a larger fan is being installed to provide additional ventilation.
Estimated completion date is January 15, 2000.
4. The fire extinguisher in the Control Building was noted to be completely
discharged.
The fire extinguisher has been replaced with a fully charged fire extinguisher.
Inspection of fire extinguishers is performed on a monthly basis and is part of
the Preventive Maintenance checklist. Previous inspection of the fire
extinguisher prior to the O&M Inspection was performed on September 10,
1999. The fire extinguisher was found to be in satisfactory condition at that
time.
Recommendations:
1. Aeration Tanks have not been rotated since Plant Start-up.
The Aeration Tanks will be rotated. Estimated completion date for bringing
Aeration Tank No. 1 on line is March 31, 2000.
2. Use of Standby Aeration Tank as flow equalization basin.
The slotted gate has been moved to the influent channel and Aeration Tank No.
1 will be pumped down to provide additional storage capacity. Pumping down of
Aeration Tank No. 1 is expected to be completed by January 31, 2000.
3. Heavy algae growth on effluent weir of Final Clarifier No. 2.
The algae growth has been removed.
