Department of Health staff inspected the subject wastewater facility on August 19, 1999. The facility is given a "Conditional Acceptance" rating based on the deficiencies cited in the report. Please respond within thirty (30) days after receipt of this letter with an affirmative action plan addressing the deficiencies noted. A resonable time schedule for the correction of the deficiencies should also be included. Hate DEC 2 2 1999 OPERATION AND MAINTENANCE INSPECTION REPORT INTRODUCTION Facility: Kapehu Wastewater Treatment Plant Date(s) of Inspection: August 19, 1999 Inspector(s): Sinn Steffany Facility Representative(s)/Job Title: Lyle Hirota/WWTP Superintendent Lawrence Ojiri/Asst. WWTP Operator BACKGROUND 1. current status of deficiencies cited in previous inapection reports: Deficiency: Laboratory analyses of BOD, loading to the plant was not available for August 1997, February 1998, and March 1998. Since BODS loading exceeded 90$ of design values for four (4) other months of the year, it is important that sampling be conducted consistently to ensure optimum plant performance. Status: Lab analyses of BOD, loading to the plant was not available for September 1998 and January 1999. BODS loading exceeded the design value during three (3) months of the year. Sources contributing to the high BOD loadings should be identified and prevented from entering the plant. Deficiency: An access cover should be provided for the seepage pit. The seepage pit should be inspected for proper functioning. Status: An access cover was not installed for the seepage pit. Deficiency: Excess solids were noted in the effluent divider boxes. Divider boxes should be cleaned regularly. Status: The divider boxes are being cleaned on a regular basis. Deficiency: Calibration records for the influent and effluent flow- meters should be available onsite. Status: The calibration records were available onsite. Deficiency: The facility is located in a residential area. For security and safety reasons the facility should either be fenced off, or have locks installed on the tank covers. Status: Locking devices were installed on the tank covers to restrict access. 2. NPDES Permit Status: Not applicable. 3. Enforcement Action: Not applicable. 4. Capacity Analysis: Average flow: 0.0098 MGD (For the period 08/x/97 to 07/10/99) Design Flow: 0.016 MGD; Percent of design capacity: 61 Previous year's: The average flow rate to the facility decreased by 27 percent when compared to last year's flow rate. Last year's average flow rate to the facility was 0.0134 MGD or 81 percent of the design capacity. The decrease in flow has been attributed to dry weather during the year. 5. Modifications/Additions - Planned/Present Construction Status: None. CIIRRENT INSPECTION 1. Reuse: The facility has a DOH approved effluent irrigation plan: Yes No 7 Not applicable 3. NPDES or Chapter 62 violations (effluent limitations, spills, etc.): None. 4. Visual quality of effluent on date of inspection (inspector's judgment): The effluent appeared to be slightly murky. 5. Laboratory Data: The following influent and effluent data were collected for the period August 1998 through July 1999: a. Influent BODS and TSS averaged 225 mg/1 and 191 mg/1, respectively. b. Effluent BODS averaged 4.89 mg/1 and effluent TSS averaged 6.46 mg/1. 6. Facility's Present Deficiencies: a. BODS loading exceeded the design value during three (3) months of the year. Sources contributing to the high BODS loadings should be identified and prevented from entering the plant; b. An access cover should be provided for the seepage pit. The seepage pit should also be inspected for proper functioning; c. The mixed liquor had a light grey color and appeared to be slightly septic. The rocks in the tanks should be inspected to ensure that adequate biological treatment is being provided for the wastewater; and d. The sewer user charges are not adequate to cover the annual operational and maintenance expenses for the Hawaii County Wastewater Treatment Facilities. SUMMARY 1. General Rating: _ ACCEPTABLE X CONDITIONAL ACCEPTANCE UNACCEPTABLE Explanation: The facility is given a "Conditional Acceptance" rating based on the deficiences noted in the report.
