B. BACKGROUND AND JUSTIFICATION (USE ADDITIONAL SHEETS AS NEEDED):
In 1998, the Office of Aging was selected to receive an award of $20,000
from the U.S. Administration on Aging to assist its Performance Outcomes
Measures Project. This project has been extended through December 31, 2000
with an additional award of $10,000. A11 expenses related to the project will
be paid from the award.

Dear Ms. Fukunaga:
The purpose of the letter is to extend the referenced letter agreement through December 3l, 2000,
and to increase the dollar allocation by S 10,000 for an amount not [o exceed $20,000. This will provide
additional time and funding ro support your continued participation in the Administration on Agin~s
Performance Outcomes Measures Project. All other provisions of the letter agreement remain in effect.
Please sign and return this amendment to Mr. Robert Ficke. Upon receipt of [his executed agreement;
Westat will send your oreanization at total payment of 520,000; 1) $10,000 for final payment of 1st year:
