. . . . . . . . . . <br />COUNTY OF HAWAVI STATE OF HAWAVI <br />RESOLUTION NO. 635 24 <br />RESOLUTION AUTHORIZING THE MAYOR TO ENTER INTO A MULTI- <br />YEAR AGREEMENT WITH DIAGNOSTIC LABS SERVICES INC. FOR <br />BLOOD COLLECTION LABORATORY SERVICES <br />WHEREAS, Section 10-11 of the County Charter requires that any contract, <br />lease or other obligation requiring payment of funds from the appropriations of a later <br />fiscal year or of more than one fiscal year be approved by resolution; and <br />WHEREAS, the Fire Department desires to enter into a multi -year agreement for <br />three (3) years with Diagnostic Labs Services Inc.; and <br />WHEREAS, the purpose of this agreement is for Diagnostic Labs Services Inc. to <br />provide blood collection laboratory services for the Hawaii Fire Department to be <br />compliant with their Bloodborne Pathogen / Infectious Disease Exposure Guidelines; <br />and <br />WHEREAS, this multi -year agreement will afford a thirty percent discount for the <br />necessary blood draw tests; now, therefore, <br />BE IT RESOLVED BY THE COUNCIL OF THE COUNTY OF HAWAI'I: <br />1. That the Mayor is hereby authorized to enter into a three (3) year agreement, <br />with Diagnostic Labs Services Inc. for the purpose as described above. <br />2. That sufficient funds be budgeted in future fiscal years to cover the <br />anticipated obligations of the County under the terms of the agreement <br />
