A high volume copier is needed for the West Hawaii program. Other services, such as the Retired & Senior Volunteer Program, Special Programs, Hawai'i County Nutrition Program, as well as the Aquatics Division and the personnel from the Fire Department at Kealakekua and Captain Cook will also utilize the copier. REOUEST: Enter into a multi-year lease with the purchase Wdtion for 5 years for (1) high volume copy machine with reduction/enlargement feature, automatic feeder, sorter, 2 paper trays, and digital model for the West Hawaii Elderly Activities Division office, 81-951 Halekii St.; Kealakekua, HI 96750. Lease to be paid from: 010-481-5481.02-111 B. BACKGROUND AND JUSTIFICATION (USE ADDITIONAL SHEETS AS NEEDED): Past practices has been to rent/lease a copy machine on a monthly basis. The basic machine without accessories at the West Hawaii Elderly Activities Division office can no longer accommodate the high demand for the amount of usage from the EAD West Hawaii staff as well as other Divisions in West Hawaii. Being Federally funded we do not have funds to purchase a copy machine, we are able to rent/lease. By renting/ leasing a model specified above would enhance staff time and efficiencv. As an example, CSE is the Information and Assistance, direct service provider, when assisting/ helping our seniors to get benefits the need to make multi-copies of forms and pertinent information to obtain a benefit is crucial. Special Programs, as well as other Programs have many forms, flyers, etc., that are needed as part of their daily work, with the added accessories it would cut staff time when collating documents/ forms, etc. The machine will be used by Coordinated Services for the Elderly, Retired & Senior Volunteer Program, Special Programs, Hawaii County Nutrition Program, Elderly Activi- ties, and at times Aquatics and Fire Department personnel from Kealakekua and Captain Cook. 01 SIGNED: h~/ DATE: ~a Depart.-men Head
