DOH, CWB 1586-4309 10-26-00 Signature of Management O A Reviewer Agency/Ofrice/Phone and Fax Numbers Data EPA Form 3580.3 (Rev 9-94) Previous editions are obsolete FINDINGS INTRODUCTION Department of Health (DOH) personnel S Sato, K Muranaka, and L Stoddard conducted an NPDES Compliance Evaluation Inspection (CEI) of the County of Hawau's (COH) Papaikou- Paukaa Wastewater Treatment Plant (PPWWTP) on September 21, 2000 Lyle Hirota, Superintendent. and Charles Dawrs, Superyisor, represented the facility dunng the inspection \PDES PERMIT NO HI0021113 was issued to the COH and became effective on January 22, 1999 and will expire at midnight on January 22, 2004 The penni[ authonzes the discharge of secondary treated wastewater from the PPW~h'TP, located m Papaikou, Hawau, to the receiving waters named the Pacific Ocean at Waipahi Point, through Outfall Senal No OOl at Latitude 19°47'09" Longitude 155°OS' l9" R', m accordance with the effluent limitations, monrtonng requvements and other conditions as set forth m the permit and m the DOH "Standard NPDES Pennrt condmons," dated October 1, 1997 FACILITY DESCRIPTION The PPWWTP is a secondary treatment plant that services the Paukaa, Puueopaku, Papaikou, and Onomea communities on the island of Hawan The plant has a design capacity of 0 35 million gallons per day (mgd) The facility's treatment processes include banninution, gnt removal, activated sludge, secondary clarification, chlorination and dechlonnahon Waste activated sludge is aerobically digested, followed by dew•atenng with a Duai Cell Gravity (DCG) unit Approximately 60 cubic feet of dew•atered sludge is transported weekly [o the COH Hilo Landfill for disposal STATUS OF PAST DEFICIENCIES The current status of deficiencies cited m last year's Operation and Maintenance inspection report include the follow•mg Deficiency The headworks venlila[ion si~stem was Inoperable For Itabtkry and personae( sujeh•, the rennlanon s}•stem should be inspected and repaired Status A larger replacement fan was purchased and the installation completed on January 5, 2000 Deftciency The chlorine a/arm x•as inoperable Status Anew chlorine alarm w•as installed on February 1, 2000 Deficiency The chlorine contact tank contained a 6 to !2-inch layer of sludge on the bortonr of the tank Sludge carryover with the final effluent can occur during peak flogs or when the sludge denitrifies Status The sludge layer on the bottom of the tank was removed Vacuuming of the tank is performed at least once per month In addition, plant personnel perform weekly sludge checks with a sludge Judge and cleaning done if necessary Deficiency The fire extinguisher in the Control Budding was noted to be completely discharged Status The fire extinguisher was replaced with a fully charged fire extinguisher Inspecnon of the facility's fire extinguishers is performed on a monthly basis RECORDS/REPORTS Revrew• of the facility's Discharge Monitoring Report data showed that the facility processed an average flow of 0 09 mgd or 26 percent of the design capacity for the one-year period September 1999 to August 2000 The flow for the prior one-year period averaged 0 105 mgd or 30 percent of the design capacity The decrease in flow is attributed to the dry weather conditions during the past year In the next few years, new sewer connections are anticipated to add approximately 0 034 mgd to the present flows The following data taverage for the past year) for the facility was compiled based on monthly averages for the period September I, 1999 to August 31, 2000 Influent BODS 205 mgrl (flow-propomonal composite sample) Effluent BODE 2 0 mg;'I (flow-proportional composite sample) Influent TSS 282 mg!I (flow-proportional composite sample) Effluent TSS 1 7 mg%I (flow-proportional composite sample) pH (min) 6 1 (grab sample) pH (max) 7 4 (grab sample) Permit hmtts are as follows PARAMETER DISCHARGE LIMITATION BODS 30 mg~l (30-day average) 45 mg/I (7-day average) 85 removal) Total Suspended Solids 30 mgil (30-day average) 45 mg/I (7-day average) 85 removal) pH Range 6 0 - 9 0 Standard Units Total Residual Chlonne 7 5 ug/I Enterococc~ 7!100 ml No v~olanons of the effluent pemut hmitahons were noted The COH submitted the facility's NPDES regmred annual reports for operations and sludge management for the 1999 calendar year There were no spills or bypasses reported by the fac~lrty dunng the past year The fac~lrty also received no odor complaints m the past year MODIFICATIONS/ADDITIONS Dunng the previous year, a manual transfer s~a•ttch and connectors to provide for the connection of a portable emergency generator was installed at the fac~l~ry Also, the fac~l~ty's Chlonne alarm was replaced with a new electronic unit with an external siren OBSERVATIONS Influent flow is measured by a Gpolleth weir and ultrasonic level sensor which is calibrated semiannually by COH mechanics The most recent calibration was performed on July 27, 2000 Influent flow samples are Flow-proportional composites taken with an automatic sampler The sampler nozzle was located near the tank wall where grease/scum was accumulating A V-notch weir and ultrasonic level sensor (m sl~lhng well) measures the effluent flow The access cover for the st~lhng well ~s bolted down and therefore the ms~de of the sUlhng well ~s not inspected by the operators It w•as recommended that the operators tnspecvclean the stilling well at the lime the mechanics perform the semiannual cal~brat~on Effluent samples are also flow- proportional composites taken with an automatic sampler Additionally, grab samples are taken for monitoring pH, total residual chlorine and enterococc~ All plant egwpment was operational Secondary clarifier \0 1 was undergoing routine overhaul work and should be completed shortly The DCG unit was not m use on the day of the mspect~on, but was reportedly operational Preventive maintenance work w•as reported to be up- to-date All treatment processes were functioning satisfactorily Plant effluent was observed to be clear SUb1~1ARY The Papa~kou Wastewater Treatment Plant ~s given a general rating of "ACCEPTABLE " The fac~hty was determined to be satisfactorily maintained and operated We have the following recommendations I Hose down the accumulated grease%scum on the walls of the tank where the influent sampler nozzle ~s located a few hours prwr to the start of the 24-hour sampling period This should result m a more representative influent sample, and 2 Inspect and clean (if necessary) the stilling well for the effluent ultrasonic sensor during each cal~brat~on event zooasa
