Box 3378 Honolulu, Hawau 96801 ~ v Subject: Kealakehe Wastewater Treatment Plant N Operation and Maintenance Inspection (EMD/WB 200671) ~ The County of Hawau Wastewater Division has received the June 23, 2000 facility inspection report and provides the following corrective action plan to address the deficiencies cited in the report 1. Reuse Projects: "The County needs to accelerate implementation of the reuse projects. To date, DOH has not received any written official response from the County " Contrary to c~mmer+ts in your inspection report, a revised construction schedule for the effluent projects was submitted to DOH on August 19,1999 along with a copy of the Effluent Reuse Master Plan Final Report dated February 1999 A copy of the cover latter and the schedule is attached for your information Phase I of the Reuse System, the Contractor Fill Station, is currently under construction and on schedule. Phase II has been delayed in order to coordinate with the future widening of the Queen Kaahumanu Highway, curcently scheduled for construction by the State DOT for FY 2001. Completed 2. Chlorinator Malfunction: "The Chlorinator has been malfunctioning since May 2000 The reuse of effluent at the "Swing Zone" driving range was terminated on May 27, 2000 " Comm. Page 2 of 4 ' For clarification, effluent reuse to the "Swing Zone" was suspended on May 27, 2000 in lieu of being terminated. Although chlorine residual regwrements for R-2 reuse regwrements were met at all times, the effluent reuse to the Swing Zone was suspended due to the fact Operators became aware of apparent equipment problems and did not want to jeopardize the quality of the effluent reuse water to the Swmg Zone Effluent reuse to the Swmg Zone was returned on July 7, 2000. With regard to the duration of the effluent reuse suspension, the extended duration was due to the fact that a series of repairs were accomplished prior to final correction of the problem, which ultimately resulted in a modification of the system. Final modification of the system resulted in elimination of the existing Chlorinator and appurtenances and replacement with a tank mounted Vacuum Regulator system The tank mounted Vacuum Regulator systems provides an additional benefit by eliminating high pressure portions of the Chlorination system. Corrective Action Completed July 7. 2000 3. Grit Classifier No. 1: "The Grit Classifier was removed from service since February 2000 due to a corrosion problem. To date, completion of the repair work on the classifier is still uncertain." Bids for repair of the Grit Classifier were solicited on May 24, 2000 with a bid opening date of June 20, 2000. No bids were received for the repair work. The item will be re-bid. Target Completion Date for the repair worts is November 2000. 4. Motor Cage of Grit Chamber Turntable Motor: "The motor cage of the grit chamber turntable motor was noted to be heavily corroded For safety and liability reasons it is recommended that the cage be repaired " The motor cage was repaired. Corrective Action Cop~pleted ust 24. 2000. 5. Effluent Pump No. 1: "Effluent Pump No 1 became inoperable on April 12, 2000. The run time of the pump was only 3,000 hours. Considering that the pumps are relatively new, the County should investigate the cause of the problem Pump No. 1 also needs to be fixed in a timely manner " The cause of the failure of the pump was due to a build-up of corrosion on the bell housing in the area of the impeller which led to binding of the impeller and overloading of the motor '?IY 1~ ' Memo OOLH-123, Kealakehe VVWTP O&M Response, EMb/WB 200651 Page 3 of 4 A Purchase Order was issued for repair and modification of the pump on May 31, 2000. Modification of the pump will include installation of a Stainless Steel wear ring in the casing, replacement of the column and lineshaft tube with Stainless Steel piping, and conversion of the pump from an oil lubncated system to a grease lubncated system. Target Completion Date is September 30. 2000 6. Safety Railings at Effluent Puma Station: "Safety railings should be provided at the stairs at the efFluent pump station." Railings conforming to HIOSH Standards were installed on the stairs and platforms located at the Effluent Pump Station. Corrective Action Completed August 25. 2000 7. Chain Links at Influent Channel: "Chain links should be installed around the influent channel to prevent personnel from accidentally falling in the channel " The area is not considered as a working platform and is not normally occupied Access to the area is controlled by a single stairway which is cordoned off with a chain across the stairway, limiting inadvertent access to the area Plant personnel discussed the issue with a HIOSH Consultation Advisor who performed a Safety Survey of the Kealakehe Wastewater Treatment Plant on May 25, 2000. The inspector indicated that controlling access at the stairway was sufficient to meet HIOSH requirements. Based on the above, please be advised that the Wastewater Division will not be installing additional barriers for the influent channel at this time. Coin I 8. General Comments/Recommendations: "It is recommended that increased ventilation be provided for the blower room. Heat could damage the blowers " The Wastewater Division will investigate the possibility of providing additional ventilation to the blower room.
