Purpose of Grant: To enable Hawaii's "na kupuna" to have access to affordable and
quality home-and-community based services that are client-centered and family-
supportive, allowing them to live with independence and dignity. Services are Adult Day
Care, Attendant Care, Case Management, Chore, Homemaker/Housekeeper, Personal
Care, Assisted Transportation, Home Delivered Meals.
