T'he 2001 State Legislature has appropriated funds to the Disability and Communication Access Board to reimburse the Counties for costs incurred in the administration (issuance) of parking permits to qualified persons with disabilities. Part of the reimbursement monies will be used to purchase computer equipment necessary to interface with the centralized statewide database. REQUEST: Amend the operating budget to include the Statewide Program on Parking for Persons with Disabilities. Amount of $23, 320 to'be transferred from state funds to the County of Hawaii. B. BACKGROUND AND JUSTIFICATION (USE ADDITIONAL SHEETS AS NEEDED The 2001 Legislature appropriated the amount of $176,000 to the Disability and Communication Access Board (DCAB) to fund the cost of the disabled persons parking placard program. Alloted amounts for each county was based on percentage distribution by island county based upon number of placards issued. Each neighbor island received $5,000 for each current issuing station (the Big Island has two issuing stations), and the remaining balance divided by percentage based upon number of placards issued, a reflection of volume and workload. This money will be put into a designated account to be used for the implementation of the statewide centralized database (server resides at the DCAB office), which will include purchase of five (5) computers and two (2) printers to be located at both issuing stations, to meet minimum requirements needed for this program. Starting January 1, 2002, all counties will be entering data from their respective stations into this centralized database. SIGNED: DATE: ~ o l Department Head MEMORANDUM OF AGREEMENT between the STATE OF HAWAII DEPARTMENT OF HEALTH DISABILITY AND COMMUNICATION ACCESS BOARD and the COUNTY OF HAWAII OFFICE OF THE MAYOR This Agreement is made this day 2001, by and between the Disability and Communication Access Board, Department of Health, State of Hawaii, hereinafter referred to as "DCAB" and the Division of Motor Vehicles and Licensing, Department of Customer Services, and County of Hawaii, hereinafter referred to as "COUNTY", to implement the Statewide Program on Parking for Persons with Disabilities, herein after referred to as "PARKING PROGRAM", which issues parking permits (placards and special license plates) and identification cards to qualified persons with disabilities who are residents of the State of Hawaii. 1. Period of Agreeme:~t: Upon the approval by the Dir~c~o~ ~f the Department of Health and the Mayor of the COUNTY, this agreement shall be effective from and shall continue through June 30, 2002, unless this Agreement is extended or sooner terminated as herein after provided. 2. Modification of Agreement: Unless terminated, this Agreement may be extended for not more than two (2) additional twelve- month periods upon mutual consent of the parties in writing at least thirty (30) days prior to expiration of this Agreement. 3. Termination of Agreement: This Agreement may. be terminated only upon mutual consent of the Executive Director of the DCAB and the Mayor of the COUNTY. Either party may terminate the Agreement, but only after sixty (60) days written notice of such intention is received by the other party. 4. Responsibilities of the COUNTY: A. The COUNTY shall issue parking placards and identification cards to qualified persons with disabilities. B. The COUNTY shall submit to the DCAB all parking permit files from existing databases operated by the COUNTY for import into the parking permit database system in the DCAB office on Oahu. C. The COUNTY shall obtain and maintain computer hardware and software and Internet service at the expense of the COUNTY to gain access to the parking permit database system. D. The COUNTY shall ensure that staff assigned to issue permits and input applicant information into the parking permit database will receive training approved and conducted by DCAB on the pertinent requirements of Title 11, Chapter 219, Hawaii Administrative Rules and on the proper use of the database system. Only staff who have completed satisfactory training shall be authorized to issue placards and input information into the database system. E. The COUNTY shall input all application and transaction information into the database system within forty-eight (48) hours or two (2) working days from the date of receipt. F. The COUNTY shall submit quarterly reports to the DCAB in the format developed by the DCAB. G. The COUNTY shall submit to the DCAB an annual projected expenditure plan by September 1, 2001, to show the projected operational expenditures of the PARKING PROGRAM for the period between July 1, 2001 to June 30, 2002. The COUNTY shall submit to the DCAB an annual budget by June 30 of each subsequent year to show the ~,roj:.cted operational expenditures of the PARKING PROGRAM for the next fiscal year beginning on July 1. H. The COUNTY shall attend quarterly meetings at the DCAB office on Oahu at the expense of the COUNTY. I. The COUNTY shall administer the PARKING PROGRAM in strict adherence to Chapter 219, Part III, Hawaii Revised Statutes and Title 11, Chapter 219, Hawaii Administrative Rules. J. The COUNTY shall conduct periodic checks to ensure that permits are issued only to individuals who present the required documentation. The COUNTY shall take appropriate corrective action in the case where issuance procedures are not strictly followed. K. The COUNTY shall ensure that the placards and expiration decals are stored securely at all issuance stations to protect them from loss, damage, or theft. 5. Responsibilities of the DCAB: A. The DCAB shall establish and maintain a parking permit database system in the DCAB office on Oahu. The database shall be accessible to the COUNTY via high- speed Internet connection. B. The DCAB shall import existing parking permit files from existing databases operated by the COUNTY into the parking permit database system in the DCAB office on Oahu. C. The DCAB shall provide approved training on the proper use of the parking permit database system to each staff person of the COUNTY assigned to entering parking permit application information onto the database. D. The DCAB shall provide parking permit database system user manuals to the COUNTY. E. The DCAB shall provide ongoing, trouble-shooting technical assistance to the COUNTY via telephone, facsimile, or e-mail with respect to the parking permit database system. F. The DCAB shall provide master copies of the parking permit application form to the COUNTY for their use to create supplies as needed. G. The DCAB shall provide identification cards or computer template to the COUNTY. H. The DCAB shall provide parking placards to the and expiration decals to the COUNTY as needed. 6. Compensation and Payment Schedule: For services performed by the COUNTY under this Agreement, the DCAB shall transfer a total sum of money not to exceed twenty three thousand three hundred twenty and no/100 ($23,320) of state funds which shall be paid in accordance with and subject to the following: A. Semi-annually upon the semi-annual submission by the COUNTY of invoices in triplicate. B. Expenditures shall be in accordance with the budget attached hereto as "Exhibit A."
