Re: Resolution Accepting Donation
Enclosed is a resolution accepting the donation of a minivan and its first year's fuel and
maintenance costs for the Department of Parks & Recreation's Elderly Activities
Division Coordinated Services Program. The minivan and its related operating costs,
are being donated by the Estate of Mr. Jack C. Fuller, P.O. Sox 6051, Ocean View,
Hawaii 96737-6051 and have a total estimated value of $25,000.

To accept and acknowledge a donation of a mini van to the Elderly Activities Division's Coordinated Services
Program
13. BACKGROUND AND JUSTIFICATION (USE ADDITIONAL SHEETS AS NEEDED):
The estate of Mr. Jack C. Fuller would like to donate amini-van to the Elderly Activities Division Coordinated
Services Program. This van will be used in Ocean View to provide seniors with local transportation for
medical appointments. The Fuller estate will supply fuel needs and maintenance for the first year.
Coordinated Services will be responsible for operating costs after the first year.
Total estimated value of donation: $25,000
