Comm. 444: RE-APPOINTMENT OF SAMUEL KAWAMURA TO THE HAWAII COUNTY
ENVIRONMENTAL MANAGEMENT COMMISSION
From Mayor Harry Kim, dated December 7, 2001, submitting for confirmation the re-
appointment of Samuel Kawamura to the Hawaii County Environmental Management
Commission. Mr. Kawamura is a lifelong resident of Hawaii and has demonstrated
integrity, diligence, and competence during his service on this Commission. He is not
affiliated with any political party and, if confirmed, will serve a term ending on
December 31, 2006.

MAINTENANCE INSPECTION REPORT

Chief, Environmental Management Division, State Department of Health, dated July
19, 2001, transmitting their operation and maintenance inspection report for the subject
facility which was given an overall "unacceptable" rating for reasons as noted in the
report.

Division, State Department of Health, from Galen Kuba, P. E., Director, Department of
Environmental Management, dated September 13, 2001 providing a corrective action
plan to address the deficiencies cited in Comm. 284.

Comm. 450: SEWER SYSTEM MAINTENANCE INSPECTION REPORTS
COUNTY OF HAWAII (EAST AND WEST HAWAII)
Copy of letter to Director of Environmental Management Galen Kuba, P. E.,
from Thomas E. Arizumi, P. E., Chief, Environmental Management Division, State
Department of Health, dated November 29, 2001, transmitting their sewer system
maintenance reports for the subject facilities . No major deficiencies were noted in the
East and West Ilawai`i sewer systems. However, noted in the reports are the
deficiencies that were cited in last year's report that still needs to be resolved.
