Enclosed for yow action is a copy of the approved highway safety reimbursement
project No. EM02-04 (OI-O-Ol), entitled "HCFD CPS Equipment." A total of
$11,160.40 has been obligated to the project.
This grant funds are to be used to pwchase child restraint seats that will be used in
ambulances in order to provide the safest transport available for children under the age
of four.
Progress reports must be submitted quarterly within 15 days (ending May, August and
November) and a final report must be submitted within thirty days of the end of the
grant period. Reimbwsement claims must be submitted in accordance with the
agreement (monthly or quarterly) by the 20th of the month following the claim period.
A claim must be submitted even if no expenditures were made.
The project is subject to the audit requirements of the Office of Management and
Budget (OM$) A-133, Audit of State2ocal Government &Non-Profit Organizations.
Note that the Department/ProgramNurnber and Title with which this project should be
associated in the Schedule of Federal Assistance should be associated in the Schedule of
Federal Assistance Programs section of yow single agency audit report are: Deparnment
of Transportation120,600 -State and Community Highway Safety Program. A copy of
all audit reports covering the grant period must be sent to the Public Affairs Office-Safe
Community Program.

the period of this grans to be issued.
