Re: Operating Budget
The U.S. Department of Health & Human Services, Centers for Disease Control and Prevention is
providing a $60,000 Public Health Preparedness and Response from Bioterrorism Grant to the
Hawaii County Fire Department for the purchase of afully-equipped trailer configured for use as a
medical response unit to mass casualty incidents as a result of weapons of mass destruction or
natural disaster.
Enclosed is a resolution to authorize the Mayor to enter into an agreement with the U.S.
Department of Health & Human Services.
These funds, passed through the State Department of Health, EMS Division, will be integrated into
the County's annual EMS ambulance contract agreement with the State.

A. REQUEST:
Prepare a resolution to authorize the mayor to enter into a grant agreement with the Center for Disease
Control, U.S. Department of Health and Human Services, passed through the State Dept of Health, EMS
Division. The grantor will provide funding of $60,000 which will be used to purchase a trailer equipped for
medical response to a mass casualty incident resulting from weapons of mass destruction or natural
disaster.

CONTRACT MODIFICATIONS:
Effective May 14, 2003, the parties mutually agree that the PROVIDER shall
continue the provision of ambulance services with the following modifications:
1. Anew paragraph I is added on page 13 of Exhibit "A," Scope of
Services to read as follows:
"I. The Contractor shall purchase a trailer configured for use as a
medical response unit for response to a mass casualty incident as a
result of weapons of mass destruction or natural disaster. The trailer
shall be equipped with personal protective and decontamination
equipment, medical supplies, nerve agent antidotes or other items
approved by the State's EMS Medical Director."
2. Exhibit "B," Compensation and Payment Schedule.
a. The total compensation amount of the Agreement is increased
by $60,000.00 of federal funds for fiscal year 2003.
