The grant amount is for $50,000. As in years past, the grant is to be spent on the purchase of various equipment and supplies to help HiCFD in meeting their goals and objectives in suppressing wildland fires. Enclosed please find two copies of a VFA Agreement, stating that HiCFD is receiving a VFA grant and entering into an agreement with DOFAW to abide by all VFA rules and regulations during the stated period. Please sign both copies and send one back to our office. Should you have any questions, please feel free to contact me at 587-4173. Ching State Protection Forester attachment ~i~~~S 43: d, ~c~Cl, F• I, ~i IG VOLUNTEER FIRE ASSISTANCE PROGRAM Grant No. 03-DG11052012-106 Phis Ageement is made and entered into by and between the Hawaii Department of Land & Natural Resources. Division of Forestry and Wildlife, as a component agency of the State of Hawaii, and the Hawaii County Fire Dept., the Grantee. The Division is awarding the Grantee a Volunteer Fire Assistance Grant for the specifics listed below and the Grantee agrees to the following: 1) The effective date of this Agreement is July 1, 2003 and expires on June 30, 2004. 2) "I'he Grant Award amount is 5$ 0,000. 3) The Grantee has one (1) year, as stated above in #l, to complete the project. 4) The money is to be spent onl y on the purchase of personal protective gear, hoses and fittings, and other supplies that are allowed under the 'volunteer Fire Assistance Progam. Purchases that were incurred prior to the effective date of the Agreement are ineligible for reimbursement. 5) The Grantee promises and agrees to follow all the rules and regulations that govern the Volunteer Fire Assistance Progam. 6) Should there be any changes to the Ageement, the Grantee shalt, in writing, request any changes no later than 30 days prior to the expiration of this Ageement. 7) The Grantee will submit aSub-Qrantee Request for Payment form no later than 30 days after the expiration of this Aeeement, alone with all supporting documents, receipts, etc. S) If, for some reason, an extension is needed, the Grantee shall, in writing, request for an extension no later than 30 days prior to the expiration of this Agreemen 9) The Grantee promises and agrees that it will indetnnify and hold harmless the Division and all of the Division's officers, agents and employees from any claim, action, damage, loss, liability or expense directly or indirectly caused by any act of the Grantee during the period of this Ageement. 10) This Ageement shall be governed by and construed in accordance with laws of the State of Hawaii. 1 ])This Agreement, when executed, embodies the entire ageement and understanding between the parties. 9 VFA Agreement Page 2 of 2 The parties hereto have caused this Agreement to be executed as written above.
