RE: "Public Health Preparedness and Response From Bioterrorism" Grant ASO Log YO-237
This is Hawaii County's invoice for the referenced grant.
The expenditures charged are for two bio-terror response trailers, one each for Kona and
Hilo. The equipment and supplies purchased are for "mass-casualty incident response" (MCI),
in lieu of "decontamination response", as was pre-approved by the State Department of Health.
I certify that expenditures chazged, as detailed on the enclosure, were incurred for the
provision of goods provided under this agreement.
Please call Gerald Makino, Accountant IV, at this address should there be concerns about
the enclosures.
