Re: Donation to the Office of Management
Enclosed is a resolution accepting a monetary donation of $800 from the Hawaii Alliance
for Community Health, P.O. Box 10508, Hilo, Hawaii 96721, These funds will be used to
offset the cost of the 2004 Hawaii Island Meth Summit III, held on October 22, 2004.
Also enclosed is a bill for an ordinance amending the Operating Budget by appropriating
the $800 monetary donation into the Office of Management's OCE account.

A. REQUEST:
A resolution to accept and acknowledge a donation of $800 from Hawaii Alliance for Community Health to the
Mayor's Office. The donors wish for the funds to be used to offset the cost of the 2004 Hawaii Island Meth
Summit III held on October 22, 2004.
Appropriate the $800.00 into the Mayor's account, 010.111.5111.02.104.
B. BACKGROUND AND JUSTIFICATION (USE ADDITIONAL SHEETS AS NEEDEDI:
Council action is being requested to accept and acknowledge a monetary donation of $800 from "Hawaii
Alliance for Community Health" to help offset the cost of putting on the 2004 Hawaii Island Meth Summit III
held on October 22, 2004 at the Waikoloa Beach Marriott Resort.
