MODIFICATIONS The following modifications are to be pertormed in accordance with all contract stipulations (specifications, delivery point, rate of delivery, period of performance, price, quantity, or other provisions by mutual action of the parties to the contract). VALIDATION OF CONTRACT MODIFICATION j 6 un~. ~.h_- Direc/f r f Health Date Asaumrar ~rer.umion ATTACHMENT Contract Modifications: Effective June 30, 2005, the parties mutually agree that the CONTRACTOR shall continue to provide glass recycling services with the following modifications: 1. Two new paragraphs 10 and 11 is added on Attachment 1, Scope of Services, to read as follows: "10. The county shall only pay for glass collected by "buy-back" operations that provide documentation to show that the glass was reused or recycled. 11. The county shall obtain documentation from "buy-back" operations to verify glass was reused or recycled. Documentation shall be kept on file, for a minimum of period of 5 yeazs, and made available to the State upon its request." 2. Delete paragraph 9, Attachment 1, Scope of Services. 3. The Time of Performance is extended from July 1, 2005 to and including June 30, 2006. 4. New paragraph 8 is added on Attachment 3, Special Provisions, to read as follows: "8. No more than 10% of funds may be used for administrative and educational purposes and to promote glass recovery, recycling, and reuse through research and demonstration projects." 5.
