• Inspection No. 309462265 -August 9, 2006, Old Dump Access Road, Kailua-Kona
- Following a meeting with the County Solid Waste Division Chief, Safety
Officer and DOH, the financial penalty of $2475 was reduced 50%.
• Inspection No. 309459535 -August 24, 2006, Leilani Street, Hilo
- No financial penalties were assessed.
• Inspection No. 309462315 -August 24, 2006, Kawaihae, Road, Kamuela
- No financial penalties were accessed.

A) The energy control procedures for isolating the energy source from the stationary engine unit to the compactor
trailer did not include locking or tagging the system which exposed employees to serious injuries when they

Several weeks ago as you remember, this division was inspected by HIOSH. There were some
issues to be corrected in each base yazd. The issues were pretty much the same and related to one
another.
Recently this week, you were given a packet which contained directions from our division chief
Mike Dworsky on instructing your employees on the following:
1) The hydraulic pressure units on the truck tractors and it's procedures with the sign-
up/acknowledgement sheets for the employees.
2) Removal of the current ladders you're using at some of the stations-{shop made pipe
ladders) and the use of pick-up/pump tracks in its place, and/or aluminum step ladders.
Along with this-the directions, procedures, and sign-up/acknowledgement sheets for the
employees.
We also had a training session with the Safety Division chief Charmaine Kamaka, Mazgazet
Ahnada, Mike Dworsky, and me. At this training session we talked about how you were to
instruct and direct your employees to "clean-out" the truck tractors with the refuse trailers and
dis-engaging the power source.
We also discussed at this meeting, the discontinued use of the pipe ladders that the employees
use to climb up onto the refuse trailers. These were deemed unsafe by HIOSH. The employees

LOCKOUT/TAGOUT PROCEDURES FOR SOLID WASTE DIVISION
STATIONARY ENGINE UNITS
1. Prior to cleaning in back of the pusher blade in the container, the stationary
engine unit shall be shut off.
2. The key to the engine sha{I be placed in operating employee's pocket and
door to stationary unit shall be closed and tagged with "Do Not Operate"
sign.
3. The employee will operate hydraulic controls to empty any residual pressure
and verify no movement of pusher blade.
4. The employee may then enter container to clean loose refuse.
TRUCK TRACTOR OR PICK UP TRUCK OPERATIONS
1. Prior to cleaning in back of the pusher blade in the container, and after
pusher blade in the container is fully extended to empty out trash at landfill,
the back door of the container shall be lowered and resting on the hydraulic
ram. Thereafter, the PTO pump and the truck engine shall be shut off before
entering container.
2. The key to the truck shall be placed in operating employee's pocket.
3. The employee will operate hydraulic controls to empty any residual pressure,
and verify no movement of pusher blade.
Q. The employee may then enter the container to clean loose refuse, and the
spotter may then clean loose refuse from the rear of the container.

ladders to gain access to the landing on the containers were exposed to serious injuries due to slips & fail hazards.
29 CFR 1910.26(c)(2)(vii) states "ladders having defects are to be marked and taken out of service until repaired by
either maintenance department or the manufacturer."

LOCKOUT/TAGOUT PROCEDURES FOR SOLID WASTE DIVISION
STATIONARY ENGINE UNITS
1. Prior to cleaning in back of the pusher blade in the container, the stationary
engine unit shall be shut off.
2. The key to the engine shall be placed in operating employee's pocket and
door to stationary unit shall be closed and tagged with "Do Not Operate"
sign.
3. The employee will operate hydraulic controls to empty any residual pressure
and verify no movement of pusher blade.
4. The employee may then enter container to clean loose refuse.
TRUCK TRACTOR OR PICK UP TRUCK OPERATIONS
1. Prior to cleaning in back of the pusher blade in the container, and after
pusher blade in the container is fully extended to empty out trash at landfill,
the back door of the container shall be lowered and resting on the hydraulic
ram. Thereafter, the PTO pump and the truck engine be shut off.
2. The key to the truck shall be placed in operating employee's pocket.
3. The employee will operate hydraulic controls to empty any residual pressure,
and verify no movement of pusher blade.
4. The employee may then enter the container to clean loose refuse, and the
spotter may then clean loose refuse from the rear of the container.

Several weeks ago as you remember, this division was inspected by HIOSH. There were some
issues to be corrected in each base yard. The issues were pretty much the same and related to one
another.
Recently this week, you were given a packet which contained directions from our division chief
Mike Dworsky on instructing your employees on the following:
1) The hydraulic pressure units on the truck tractors and it's procedures with the sign-
up/acknowledgement sheets for the employees.
2) Removal of the current ladders you're using at some of the stations--(shop made pipe
ladders) and the use of pick-up/pump trucks in its place, and/or aluminum step ladders.
Along with this-the directions, procedures, and sign-up/acknowledgement sheets for the
employees.
We also had a training session with the Safety Division chief Charmaine Kamaka, Margaret
Ahnada, Mike Dworsky, and me. At this training session we talked about how you were to
instruct and direct your employees to "clean-out" the truck tractors with the refuse trailers and
dis-engaging the power source.
We also discussed at this meeting, the discontinued use of the pipe ladders that the employees
use to climb up onto the refuse trailers. These were deemed unsafe by HIOSH. The employees

were to be directed by you to use either the pick-up truck they're driving or the aluminum
ladders that was to be provided.
We agreed at this meeting to take corrective action. Part of this corrective action was to train,
direct, and instruct out employees to the corrective measures and document that this training
took place by means of having our employees signing an acknowledgement form indicating that
they have been trained, instructed and understood what the training was about. This was to be
carried out by you, our district supervisors.
To comply with HIOSH, we need these training done immediately on the above issues. We have
a specific time frame to do this. So to comply within this timeframe I will need your training and
acknowledgement forms back to this division office to Mike Dworsky no later than Tuesday next
week the 31 You shall fax the responses to this office and pouch back the originals to this
office also. Keep a copy for your files at your office. This procedure will make sure you can train
everyone that's in your base yazd. You will see them at least once.
For those that are off work due to illness, vacation, or any type leave, you will instruct them as
soon as they return to work and send to this office the necessary paperwork that's been
requested.
Do not fail this requirement. This division does not want to pay a fine for this citation if you fail
to train our employees. The fine could be a considerable amount. Please be on top of this and do
it immediately.
