Subject: Papaikou Survey -Right to Entry

The County of Hawaii has entered into a contract with BK Engineering to design an extension to
the sewer system which would service the subject property. In order to complete the design of
this project, the designer requires to perform a topographic survey of the area. We are
requesting your permission to enter the subject property for survey purposes. Attached you will
find two copies of this document. Please return one signed copy to:
