RESOLUTION NO. 512 OS
A RESOLUTION TRANSFERRING/APPROPRIATING AN APPROPRIATION OUT
AND FROM THE DESIGNATED FUND ACCOUNT(S) AND CREDITING SAME TO
A DESIGNATED FUND ACCOUNT(S)
WHEREAS, the Bay Clinic, Inc has requested funds to conduct a Diabetes Self
Management Education Program; and
WHEREAS, Bay Clinic, Ina (BCI) is a nonprofit community health centers
providing low-cost health care to the uninsured, underinsured and low-income residents of
East and South Hawaii Island; and
WHEREAS, the program is geared towards helping patients control the negative
health effects of diabetes in the aging native Hawaiian communities in Hilo and Keaukaha;
and
WHEREAS, Contingency Relief Funds from Council District 4 will be appropriated
to the Office of Aging to be used towards the Diabetes Self Management Education Program;
now, therefore,
BE IT RESOLVED BY THE COUNCIL OF THE COUNTY OF HAWAII that
the following amount is hereby transferred appropriated as set forth below:

BE IT FINALLY RESOLVED that the County Clerk shall forward a copy of this
resolution to the Executive on Aging and Director of Finance and that the Director of Finance
is hereby authorized to make the necessary transfers in accordance with the terms of this
resolution.
