This is to ir0orm you that on March 22, 1995, 1 made elevation measurements at your
well site on Lot 17-C of Ld. Ct. Cons. 117, Map 86.
1 determined the elevation of an angle iron bracket welded to the casing pipe 1.05 feet
below the to? to be 653.00 feet M.S.L.
This elevation is measured from the Kohala Ranch Project 1 bench mark located at the
centerline intersection Sta. 38+83.66: Road "C" = Sta. 0+00: Road "M" (intersection of
Palila and Olomana Streets). The elevation datum is Mean Sea Level.

In accordance with the Department of Land and Natural Resources Administrative
Rules, Section 13-168, entitled -Water use, wells. and Stream Diversion Works', your
application to construct, test, and install a pump in Wailta Well (Well No. 6450-04) within
Tax Map Ke)r . 5-9-08:7, fog-r'arigaQZt:seT is approved subject to the following conditions:
1. The Division of Water Resource Management (DWRM), Geology-Hydrology
Section, shall be notified at S48-7543, before any work covered by this
permir commences.
2. The proposed use shall not adversely affect existing legal uses in the area.
3. The grouted annulus of the well' shall be from 0 to 500 ft. instead of from 0
to 50 ft. as proposed, and the casing shall be designed accordingly.
4. The following shall be submitted to DWRNI, P.O. Box 373, Honolulu, Hawaii
96809, within 30 days after completion of the well:
a. Well Completion Report.
Ii. Elevation (referenced to ocean sea level) survey by a Hawaii-licensed
surveyor.
c. As-built sectional drawing of the well.

d. Plot plan and map the exact location of the well.
e. Complete pwuping t^~: wcoid, including time, pumping rate,
drawdown, chloride content, and water quality data.
S. The applicant shall comply with all applicable laws, rules, and ordinances.
6. The applicant shall provide and maintain an approved meter or other
appropriate device or means for measuring and reporting total water usage
on a monthly basis.
7. This permit may be revoked if work is not started within six months of the
date of issuance or if work is suspended or abandoned for six months. The
work shall be completed within two years of the date of issuance.

5. The applicant shall comply with all applicable laws, rules, and ordinances.
6. The permit may be revoked if work is not started within six (6) months after the date of
issim cc or if work is suspended or abandoned for six (6) months, unless otherwise
specified. The work proposed in the well construction permit application shall be
completed within two (2) years from the date of permit approval, unless otherwise
spocifAd. The permit may be extended by the Commission upon a showing of good
cause and good-faith performance. A request to extend the permit shall be submitted to
the Commission no later than three (3) months prior to the date the permit expires. If the
commencement or completion date is not met, the Commission may revoke the permit
after Ipving the pemmree notice of the proposed action and an opportunity to be heard.

Section 13-163, entitled "Water Use, Welts, and Stream Diversion Works", your application to

area, including any surface water or established instream flow standards. This permit or
the awhorizarion to construct and puwp water from the well shall not constitute a
detertrination of correlative water fight,. The permittee is notified and by this provision
understands that the quantity of water taken from the well could be reduced by the
Corrnroission in the future. This permit is not a commitment that the pump capacity
permitted here or even come lesser a[ii?unt is guaranteed in the future.
3. l'he applicant shall provide and rnawtutl ind approved meter or other appropriate device
