WHAT TO KNOW ABOUT COLLECTING SIGNATURES
4
,,The identification code in the top right hand corner must be the same code throughout that attached set
of papers,

* You are to make the code up yourself, and

*Change the code by at least one digit on the next separate packet of signature papers.

*The affidavit that you will sign when the packet is complete will require that you fill in the identification
code from the top right hand corner starting with Page 6, so each packet needs to have the same 'code'
throughout it's pages, and that is the code that you will fill the blank in with on the affidavit.

*When you complete a packet, fill out the affidavit provided to you, and take it to a bank or a public notary,
attach the affidavit to the signature packet and have it `Notarized; After it has been notarized, then take it
to the County Clerk's office, located in the Ben Franklin building on Kilauea in downtown Hilo, and submit
the notarized packet to the County Clerk(Casey Jarman), or the office assistant (Pauline or Val) to give to
Casey. If you do not have a bank that will 'Notarize' for free, then call Adam at 854 - 4230 and we will be
happy, to re-imburse you, or provide payment for the cost up front if its needed; please do keep your
receipt, and keep in mind that we request that volunteers collecting less than 45 signatures provide or pay
for their own notarization whenever possible.

*Be sure to fill in how many signatures are on each page.

*If your signature packet doesn't have 16 pages, then draw a line through the page numbers that aren't in
your packet. For example, if you only had a packet with one page of signatures, then at the bottom of the
signature page(Page 6), fill in the total number of signatures and put this number on the affidavit for Page
6.
Then put a single line through the other statements-'Page 7 contains a total number of signatures'
'Page 8 contains a total number of signatures'
` Etc....

*This is important! - If your signature packet has pages that do not have signatures on them, and you are
unable to collect those signatures, or you are just ready to hand a packet of signatures in for another
reason, do NOT cross those pages out, or put a line through them, simply write 0(zero) at the bottom of
the page and on the affidavit where that page is indicated.

Take a good look at what the affidavit is attesting, because it is what you are attesting, and I recommend
to act in a way that you can sign the affidavit with a clean conscious.

County Clerk - Casey Jarman
County Clerk's Office - 961 - 8255
Ben Franklin Building, 2"d Floor, C_
333 Kilauea Avenue, Suite 209 `
Hilo, HI 96720

Adam Lehmann
Project Peaceful Sky F
(808)854- 4230
projpeacefulsky@care2.com

Comm.
