Goals: Older adults have in-home and community-based long term care options. Objectives: For each service year, at least 75% of Kupuna Care clients surveyed report that their needs were met and that they remained in the home at Least three months due fo Kupuna Care services. Outcomes or Results: 89.3% of Kupuna Care Program clients reported that their needs were met and 82.1% remained in their homes at least three months as a result of Kupuna Care Program services.
