inspection report for the subject facility. The facility is
given an "ACCEPTABLE" rating. No major deficiencies were noted.

OPERATION AND MAINTENANCE INSPECTION REPORT
INTRODUCTION

BACKGROUND
1. Current status of deficiencies cited in previous inspection reports:
Deficiencies that were noted in the previous inspection are as follows:
1. The annual certification of the crane and hoist at the facility had
expired;
Certification of the crane and hoist were done on 8/14/97.
2. Three violations of the NPDES effluent pH minimum limit.
No violations of the NPDES permit since 2197.
2. NPDES Permit Status:
Permit Expiration Date: June 30, 1996
The facility's NPDES permit No. HI 0021113 was administratively extended
by the Clean Water Branch on September 23, 1996.
3. Enforcement Action:
None.
4. Capacity Analysis:
Average flow: 0.087 MGD (For the period 711197 to 6130198)
Design Flow: 0.350 MGD; Percent of design capacity: 25%
Previous year's comparison: There was a decrease of 0.012 MGD from the
previous year's average of 0.099 MGD.
5. Modifications/Additions - Planned/Present Construction Status:
None.
CURRENT INSPECTION

4. Visual quality of effluent on date of inspection (inspector's judgment):
The effluent was noted to be clear on the date of the inspection.
5. Laboratory Data:
The following influent and effluent data were collected for the period
July 1997 through June 1998:
a. Influent BOD5 and TSS averaged 183 mg/l and 198 mg/1, respectively.
b. Effluent BOD5 averaged 2.4 mg/l and effluent TSS averaged 2.5 mg/1.
6. Facility's Present Deficiencies:
1. Excessive rust was noted on the blowers.
2. The effluent flow meter was reportedly calibrated but did not have
a calibration tag.

SUMMARY
1. General Rating:
X ACCEPTABLE CONDITIONAL ACCEPTANCE UNACCEPTABLE
Explanation: The facility is being operated and maintained
satisfactorily.
2. General Comments/Recommendations:
1. The chlorine contact tank (CCT) had excessive solids at the tank's bottom.
Denitrified sludge was also noted floating on the treated effluent in the
CCT. The tank should be cleaned out more often to prevent solids carryover
in the effluent.
