BE IT ORDAINED BY THE COUNCIL OF THE COUNTY OF HAWAII:

To Increase $30,000 in Revenue Account 3301.15 for Coordinated Services.
To appropriate the $30,OOG to expend funds in:.

B. BACKGROUND AND JUSTIFICATION (USE ADDITIONAL SHEETS AS NEEDED):
Additional Federal Title 36/3C funds for FY 1999-2000 of $30,OOO,is available
to replace a 15-passenger van for the Kona district. The van will be used. to
-serve West Hawaii/Kona Region by providing transportation services to over 450
seniors yearly, which includes over 50 HCdP participants. The number of trips
per month would amount to over 500 with priority given to medical, essential
shopping, assisted transportation, and others such as recreational, educational,
and other support services.

SUBJECT: Coordinated Services for the Elder~y~('STL) 'Vehicle Purchase
We will be submitting the request of $30,000 to The State Executive Office on Aging to replace a
15 passenger van for the Kona district. Because these funds must be encumbered by
June 30, 2000, it may be prudent for CSE to initiate U}e following:
1) Contact the Purchasing Office and find out if the vehicle you plan to purchase is
on the County's bid fist. If it is not on the bid list, contact Bill Gray and request a
special bid and explain the June 30 deadline.
2) Submit a Request for Council Action (Form 8-52) to the Finance Department to
increase CSE's budget by $30,000.
