Re: Resolution Accepting Donation
The Dr. Richard Mamiya Golf Classic is held each year to honor retired Honolulu
Cardiologist Dr. Mamiya and to benefit the American Heart Association. For the past three
events, a portion of the tournament's proceeds was designated to donate Automated
External Defibrillators (AED) to the Big Island community in Dr. Mamiya's name. Three (3)
Automated External Defibrillators, valued at $6,095.72, are being donated to the
Department of Parks and Recreation and will be placed at the Afook-Chinen Civic Center,
the Edith Kanaka'ole Tennis Stadium, and the Pana'ewa Rainforest Zoo.
Enclosed is a resolution accepting the Automated External Defibrillators from Richard T.
Mamiya M.D., Queen's Physician Office Building, 1380 Lusitana Street, Suite 710,
Honolulu, Hawai'i 96813.

A. REQUEST:
To accept and acknowledge a donation of three Automated External Defibrilators (AED) to be located at
Afook-Chinen Civic Center, Edith Kanakaole Tennis Stadium, and the Panaewa Zoo.
B. BACKGROUND AND JUSTIFICATION (USE ADDITIONAL SHEETS AS NEEDED):
Council action is being requested to accept and acknowledge a donation of three Automated External
Defibrillators (AED) valued at $6,095.72 by Dr. Richard Mamiya. Additional background information attached.

RE: Resolution No. 288-09 A Resolution Authorizing the Acceptance of a Donation of
Three (3) Automated External Defibrillators to the Department of Parks and Recreation
Pursuant to Section 2(g) of Rule 4 of the Rules of Procedure of the Council of the County of
Hawaii, this written request is submitted with my approval that the above-referenced matter be
waived from the Finance Committee to the full Council for immediate action. In the event this
request is denied, for whatever reason, I understand the matter shall be referred to the Finance
Committee for placement on its fixture agenda.
