SUBJECT: INTENT TO AWARD AMERICAN RECOVERY AND REINVESTMENT
• ACT OF 2009 FUNDS FOR CHRONIC DISEASE SELF MANAGEMENT

Subject to the availability of funds, the Executive Office on Aging (EOA) intends to award the
County of Hawaii, Hawaii County Office of Aging grant funds from the Recovery and
• Reinvestment Act of 2009 for the Chronic Disease Self Management Program (CDSMP).
The funds in the amount of $25,496.00 will be available from the date on the State's Notice to
Proceed through March 30, 2012.
These funds are subject to allotments to be made by the Director of Finance, State of Hawaii
pursuant to Chapter 37, Hawaii Revised Statutes. In addition, this award is subject to the
Recovery Act terms and conditions and Reporting Requirements.
