RESOLUTION NO. 395 10
RESOLUTION AUTHORIZING THE OFFICE OF THE MAYOR TO ENTER
INTO AN AGREEMENT WITH THE STATE OF HAWAII DEPARTMENT
OF HEALTH, PURSUANT TO HRS 46 -7, FOR THE CHRONIC DISEASE
SELF MANAGEMENT PROGRAM
WHEREAS, the State of Hawaii Department of Health has authorized funding to
the Hawai'i County's Office of Aging for its Chronic Disease Self Management Program
(CDSMP); and
WHEREAS, These funds, derived from the American Recovery and Reinvestment
Act of 2009 (ARRA), will be used to expand the CDSMP workshops to Kohala and
Hamakua; and
WHEREAS, the CDSMP is an evidenced -based prevention program that closely
replicates a specific intervention that has been tested through randomly controlled
experiments with published results; and
WHEREAS, the goals of this project is to build an Aging Network capacity in
evidenced based (EB) programming and to embed the Chronic Disease Self
Management Program in the Aging Network.
NOW, THEREFORE, BE IT RESOLVED BY THE COUNCIL OF THE COUNTY
OF HAWAII, in accordance with section 46 -7, Hawaii Revised Statutes, that the Mayor
of the County of Hawaii is authorized to execute, on behalf of the County, an agreement
with the State of Hawaii Department of Health, to enable the County of Hawaii to
expand the Chronic Disease Self Management Program, as described in the scope of
services submitted in their proposal to the State, attached hereto and incorporated
herein by reference as Exhibit "A ".
BE IT FURTHER RESOLVED that the Mayor of the County of Hawaii is
authorized to sign the above - referenced agreement and any related documents on
behalf of the Office of the Mayor and the County of Hawaii pursuant to the Hawaii
County Charter.
BE IT FURTHER RESOLVED that the County Clerk of the County of Hawaii shall
transmit copies of this resolution to the Office of the Mayor, the Department of Finance,
and the Office of Aging.

Chronic Disease Self Management Program — ARRA funds
Draft Scope of Service
a. Evaluate trainee mastery and readiness to implement CDSMP, DSMP, and /or ASMP.
b. Utilize the Aging and Disability Resource Center ( "ADRC ") to identify older adults with
chronic diseases who will be offered CDSMP, DSMP, and /or ASMP.
c. Offer six (6) cycles of CDSMP, ASMP, and /or DSMP. Engage a sufficient number of
participants in CDSMP, ASMP, and /or DSMP so that, during the period of Contract
performance, at least sixty (60) participants complete at least four (4) out of the six (6)
sessions that make up a cycle.
d. Identify sites for CDSMP, ASMP, and /or DSMP workshops in the targeted communities
of Kohala and Hamakua.
e. Recruit and train Lay Leaders to deliver workshops with fidelity in the targeted
communities.
f. Identify sites for CDSMP, ASMP, and /or DSMP workshops in the targeted communities
of Kohala and Hamakua.
g. Recruit and train Lay Leaders to deliver workshops with fidelity in the targeted
communities.
h. Monitor with fidelity the workshops delivered.
i. Participate in evaluation activities with the STATE's contracted evaluator.
j. Develop plans to sustain CDSMP, ASMP, and DSMP in selected communities by
working with local and statewide HAP committees.
