Grant Period (Commencement & Completion): July 1, 2009 to June 30, 2011
Goals: To enhance each clients' access to affordable and quality home and community -based
services that are client - centered and family- supportive, allowing them to live with independence and
dignity in their own homes and communities.
Objectives: At least 75% of Kupuna Care clients will remain in their homes at least three months
and report that their needs were met.
Outcomes or Results: 81% of Kupuna Care clients remained in their homes for at least three months
reducing admittance rates to Nursing Facilities and/or Long Term Care Institutions. 83% of Kupuna
Care clients reported that their needs were met by the program throughout the two year grant period.
